Children entering Kindergarten must be five (5) years of age on or before September 1st, and students entering First Grade must be six (6) years of age by September 1st.
Each applicant K-8, is assessed to ensure a smooth and appropriate transition into OLM School's programs. This appointment is made through the School Office.
Please submit the following forms with your completed application:
1. Birth Certificate
2. Immunization Records
3. Most recent Report Card (Grades 1 - 8)
4. Baptismal/Sacramental Certificates (if applicable)
There is a Non-Refundable Application Fee of $50.00
